Liability Waiver, Release, and Assumption of Risk

I hereby agree to abide by all rules and regulations set forth by NXTLVL Sportz, LLC for participation in the above-referenced amateur sport league or tournament. I further agree to release, waive, discharge, and hold harmless the organizers, NXTLVL Sportz LLC, Impact U LLC, Gold’s Gym McAllen, Gold’s Gym, all sponsors, supervisors, volunteers, officials, and participants from any and all liability for injuries, illnesses (known or unknown), damages, or losses that may occur as a result of participation in the league or tournament.

I understand and acknowledge that participation in the NXTLVL Sportz, LLC 5v5 or 7v7 Football, Basketball, and Volleyball League or Tournament involves inherent risks, including but not limited to sprains, twisted ankles, being struck by a ball, jammed fingers, and more serious injuries or illnesses. I knowingly and voluntarily assume all such risks, whether foreseen or unforeseen.

I hereby waive, release, absolve, indemnify, and agree to hold harmless NXTLVL Sportz, LLC, Impact U LLC, Gold’s Gym McAllen, Gold’s Gym, sponsors, and any other individual, firm, or organization associated with the league or tournament from any claim, demand, action, or cause of action arising out of or related to participation, including acts or omissions by any of the above parties.

This waiver and release shall be binding upon my heirs, legatees, administrators, assigns, and personal representatives. I further agree to hold harmless the above-mentioned entities from any liability, loss, cost, or expense (including but not limited to attorney’s fees, medical expenses, and ambulance costs) that may be incurred as a result of participation.

I understand that registration fees are non-refundable once the season has started.

In the event of an emergency, I hereby grant permission for emergency medical treatment to be administered as deemed necessary.

This waiver shall remain valid unless canceled or modified in writing by the undersigned participant, parent, or legal guardian.

Additionally, I grant full permission to NXTLVL Sportz, LLC and all associated parties to use photographs, videos, motion pictures, recordings, or any other media taken during the event for legitimate purposes, including but not limited to promotions, marketing, website content, and social media.
